Qualifying Conditions

These are the qualifying conditions for the Medical Cannabis Program as set forth by the New Mexico Department of Health:

  1. Amyotrophic Lateral Sclerosis (ALS)
  2. Cancer
  3. Crohn's Disease
  4. Damage to the nervous tissue of the spinal cord, with (proof of objective neurological indication of intractable spasticity required)
  5. Epilepsy
  6. Glaucoma
  7. Hepatitis C Infection currently receiving antiviral treatment (proof of current anti-viral treatment required)
  8. HIV / AIDS
  9. Hospice Care
  10. Huntington's Disease
  11. Inclusion Body Myositis
  12. Inflammatory Autoimmune‐mediated Arthritis
  13. Intractable Nausea/Vomiting
  14. Multiple Sclerosis
  15. Painful Peripheral Neuropathy
  16. Parkinson’s Disease
  17. Post‐Traumatic Stress Disorder (PTSD)
  18. Severe Anorexia/Cachexia
  19. Severe Chronic Pain
  20. Spasmodic Torticollis (Cervical Dystonia)
  21. Ulcerative Colitis

The State requires the patient have a Valid NM issued Photo ID or Driver’s License; Temporary or Extension IDs are not accepted.

The State also requires current copies or your medical records supporting the condition you are applying under.  If you do not have copies of your records, San Juan Medical Cannabis Center can obtain the for you.  Please call us at 505.327.0002 or stop by 608 W. Animas St. in Farmington, Monday through Friday between 10:00am - 2:pm.
